Participant Waiver and Release of Liability

Please read this document carefully. By signing below, you acknowledge and agree to the terms outlined in this waiver.

1. Assumption of Risk
I, the undersigned participant, acknowledge and understand that participating in activities such as Brazilian Jiu-Jitsu, Martial Arts, Yoga, and using the gym facilities at Real Movement and Martial Arts (a division of Real Wellness Corporation) involves physical activity, exertion, and potential risks, including but not limited to injury, strains, sprains, fractures, or other physical harm.

2. Release of Liability
In consideration of being allowed to participate in the activities offered by Real Movement and Martial Arts, I, on behalf of myself, my heirs, and legal representatives, hereby release and hold harmless Real Wellness Corporation, Real Movement and Martial Arts, their employees, officers, agents, instructors, contractors, and affiliates from any and all claims, liabilities, injuries, or damages arising out of my participation in any activity or use of the gym and facilities.

3. Medical Clearance
I represent that I am physically fit and capable of engaging in the activities provided by Real Movement and Martial Arts. I agree to consult with a medical professional prior to engaging in any physical activities if I have any doubts about my physical condition. I also understand that I am responsible for ensuring I have medical insurance to cover any injuries that may occur during my participation.

4. Emergency Medical Treatment
In the event of an emergency, I consent to the administration of first aid or other medical treatment deemed necessary by qualified medical personnel. I understand that Real Wellness Corporation and Real Movement and Martial Arts are not responsible for any medical costs associated with treatment.

5. Photo/Video Release
I hereby grant permission for Real Wellness Corporation and Real Movement and Martial Arts to take photographs and videos of me during my participation in classes or events and to use such images for promotional or marketing purposes, including social media, website, and other promotional materials.

6. Compliance with Rules
I agree to follow all rules and instructions provided by the instructors and staff at Real Movement and Martial Arts. I understand that failure to comply with the rules may result in my expulsion from the premises or suspension from classes.

7. Governing Law
This waiver shall be governed by and construed in accordance with the laws of the state in which the facility is located. Any disputes arising under or in connection with this waiver shall be resolved in the appropriate jurisdiction.

8. Severability
If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions will continue in full force and effect.

9. Acknowledgment of Understanding
I have read and fully understand the contents of this waiver. By signing below, I acknowledge and agree to the terms and conditions outlined above.
